We are a membership primary care practice made directly between the patient and the physician. Unlike typical primary care offices that have a few thousand patients, our goal is to have a limited panel (a few hundred). This allows us to give unlimited access and personal service to all our patients. This individualized approach has been shown to lower emergency room use, hospitalizations, exposure to possibly contaminated crowded waiting rooms, as well as maintain healthier lives. We also give 24/7 access via phone to your physician outside of scheduled office hours.

We do not file any claims with insurance. While you do not need insurance to see us, we highly recommend having insurance to not only to handle catastrophic events but to use for specialists, labs, and imaging.

Our fee includes an annual physical as well as any in office or virtual visits. We also offer free in-office phlebotomy for labs we order.

We only charge for services that require medication injections or supplies. Our time is included in the membership fee. We also offer reduced priced labs for those who might not have insurance.

There are no copays.

There are no contracts. You are free to cancel your membership with a 30-day notice.

We see ages 18 and up.

All referrals, labs, and imaging will be to providers of your choice and/or dictated by your insurance. Again, we recommend having insurance to accommodate these services.

If you need hospitalization, you will be admitted by a hospitalist affiliated with that hospital. While we do not treat patients in the hospital, we will have access to your hospital records and will coordinate any follow up care.

We prefer to see our patients in the office or virtually. We can make house calls within a 10-mile radius in the event you are not able to come into the office but it will incur a small service charge.

All visits require an appointment. However, for ill-visits we offer same day or next day appointments.

We encourage you to meet your private concierge physician prior to joining to ensure both you and our office make a good fit to handle your conditions.

Unfortunately, direct primary care memberships at this time do not count towards your deductible and do not qualify as an FSA or HSA expense. This legislation is currently under debate and may change in the future.

We do not accept patients with these insurances. They are highly regulated and require patients to have a PCP that is within their network.

We can see Medicare patients but do not file any claims to Medicare. For us to see you, you will need to sign a Medicare Opt-out agreement stating that neither you nor Ponte Vedra Concierge will file any claims to Medicare. You may still use Medicare for diagnostic testing and referrals.

There will be times that our physician may be sick or on vacation. We will give our patients as much notice as possible and ensure that there is coverage for any of your needs.

We take most major credit cards or check if paying for the whole year.
